Jack died at the Fairfield-Lancaster Hospital, his wife Mildred signed his death certificate. His death certificate showed him as JACK

His wife Mildred died at the Riverside Methodist Hospital, Columbus, Ohio. Her son Edwin Denney signed her death certificate.

Per his grandson David Edwin Denney, his name was Ivan Denney which I did have listed in his father's family, he did not like the name Ivan and changed it to Jack. This has caused some confusion to family history buffs as they see the name different ways
